Myotonic dystrophy (DM) is a slowly progressing multisystemic disorder, characterized by a triad of myotonia, muscle weakness and cataracts. Anaesthetic concerns in patients with DM, especially in those with myotonic dystrophy type 1 (DM1) and myotonic dystrophy type 2 (DM2), are not well documented. The aim of this study was to investigate the anaesthetic management of these patients and to evaluate their perioperative outcome. Twenty-six patients with DM (13 DM1 and 13 DM2) were included in this prospective study. All patients were evaluated before anaesthesia to exclude associated conditions potentially contraindicating anaesthesia, such as severe cardiac disease, severe COPD, severe sleep apnoea, liver or renal dysfunction and hypothyroidism. Twenty patients (76%) received general anaesthesia (GA), six (23%) regional anaesthesia (RA) and zero local anaesthesia. Among the patients receiving GA, the anaesthesia was induced with etomidate, fentanyl, suxamethonium and vecuronium. After intubation the lungs were ventilated using a mixture of oxygen and air. The trachea was extubated after loss of consciousness. The mean time for loss of consciousness was 10.8+/-5.4 min (range 5-27 min). The mean time to tracheal extubation was 6.2+/-3.8 min (range 3-15 min). The mean recovery time was 26.8+/-9.6 min (range 15-50 min). Four patients (15%) complained of moderate postoperative discomfort. Eight patients (30%) were discharged from the post-anaesthesia care unit after 1.1+/-0.5 h (range 1-4 h). In
